Select records by intent

Do not review every record. Choose records that contain decisions, learnings, or open loops.

Each selected record has a reason.
Low-value noise is left out.

Extract decisions and questions

Separate what was decided from what still needs clarification.

Decisions are written as statements.
Open questions are visible.

Create the journal entry

Use a reflective template when the goal is self-review; use an operational template when the goal is execution.

The entry has a clear date or context.
Action items are not buried in prose.
